A.   INSTRUCTIONS
	For immediate processing renew online today at aom.org. Please make all corrections to your personal information directly on this invoice in Section 1. Please print or type. The SUB-TOTAL AMOUNT DUE in Section 2 is based on your membership status and division choices from the prior billing cycle. If you wish to change divisions listed in Section 2 please cross out the old division choices and write in the new division choices. Use the division table below for reference. Please be sure to select your two complimentary divisions. If division selections change your TOTAL AMOUNT DUE, please recalculate your total and submit additional payment. Please fill out Section 4 completely. You must remit this form with your payment to the address on reverse side. Retain a copy for your records.
B.   MEMBER TYPES AND FEES:
ACADEMIC:   $ 214.00
	A teacher and/or researcher of management in a college, university or research institute.
EXECUTIVE:   $ 214.00
	A management consultant or other management practitioner. A manager is defined as an individual who spends over fifty percent of his time or her time in non-academic professional management.
STUDENT:   $ 106.00 (for up to six years)
	A graduate student with an interest in the field of management who is not otherwise employed on a full-time basis. Dues are at a reduced rate of $ 106.00 for a maximum of six consecutive years or until graduation, whichever comes first. PROOF OF STUDENT STATUS IS REQUIRED. Falsification of membership status is a violation of the Academy's code of ethical conduct; adjusted fees will be applied.
EMERITUS:   $ 106.00
	Emeritus status is for individuals who are retired without full-time employment AND who have been members of the Academy for at least 10 consecutive years preceding qualification as emeritus.
C.   DIVISIONS AND INTEREST GROUPS:
	Your dues renewal payment includes two divisions or interests groups. Additional divisions are $ 11.00 each, and additional interest groups are $ 7.00 each. Record your choices with payment in Section 3 on the reverse side.
